This notice describes how medical information about your child may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment

Luma Pediatrics is committed to protecting your child's medical information. We are required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of protected health information ("PHI"), provide this Notice of our legal duties and privacy practices, and follow the terms currently in effect.

How we use & disclose PHI

  • Treatment. To provide and coordinate your child's care.
  • Payment. To bill insurance and obtain payment for services rendered.
  • Healthcare operations. To run our practice, evaluate quality, and train staff.
  • Required by law. Public health activities, reporting abuse/neglect, judicial orders, and similar mandates.
  • With your authorization. Any other use or disclosure not described above will require your written authorization, which you may revoke at any time.

Your rights

  • Access. Request a copy of your child's medical records.
  • Amend. Request corrections to information you believe is incorrect or incomplete.
  • Accounting of disclosures. Receive a list of certain disclosures we've made.
  • Restrict use. Request limits on how we use or share information for treatment, payment, or operations.
  • Confidential communications. Request that we contact you in a specific way (e.g., by mail rather than phone).
  • Paper copy. Request a paper copy of this Notice at any time.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Luma Pediatrics or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate for filing a complaint.
