Stay connected

Bring three things
Photo ID, your insurance card, and your child's immunization records (if available). For newborns, bring discharge paperwork from the hospital.

Meet the team
Plan on about an hour for a first visit. We take time to listen, do a thorough exam, and answer every question you have.

Leave with a clear plan
After opening, our planned visit experience includes clear summaries, next steps, and appropriate office or telehealth follow-up.
A quick checklist
- Photo ID for the parent or legal guardian
- Insurance card
- Immunization records, if available
- A list of any current medications
- For newborns: hospital discharge paperwork
- Forms that need to be signed (school, sports, camp)

We will publish accepted plans before scheduling opens. Self-pay families will also be welcome, with pricing discussed transparently up front.
